Norwin School District will once again be offering free sports physicals for any student entering grades 7th through 12th wishing to try out for a school sponsored sport for the 2017-2018 school year. Physicals will be overseen by Daniel Medic, D.O. with the assistance of residents from Seton Hill University and our Norwin athletic trainers. These physicals will be held on Thursday June 1, 2017 at Norwin High School.
In order to receive a free physical students and parents must complete Sections 1-5 of the CIPPE, the Acknowledgement of Receipt of Notice, and Consent for Treatment forms in the physical packet. Packets must be completed in their entirety prior to receiving a physical and it is preferred that it be submitted to the Athletic Department before Thursday June 1, 2017.

Once again, please make sure that all pages are completed in their ENTIRETY; all pages are front and back. Any incomplete packets will be returned to the student. (NO faxed or old forms will be accepted.)
Appointment times are assigned based on the student’s grade for the 2017-2018 school year. Families with multiple student athletes should come to the earliest applicable time slot(s). ImPACT Baseline Testing will be conducted immediately following the student’s physical for all students entering 7th, 9th, and 11th grades and any student who was not tested last school year for a sport.
7th & 8th Grade- 8:00am
9th Grade- 9:00am
10th Grade- 9:45am
11th Grade- 10:30am
12th Grade- 11:15am
If students wish to receive a sports physical with their family physician, the physician must complete and sign Section 6 of the PIAA CIPPE form enclosed in this packet. These physicals must be dated June 1, 2017 or later. The physical packet must be completed in its entirety and submitted at least 1 week prior to the start of the sports season. Forms completed over the summer by a private physician should be submitted to the NORWIN HIGH SCHOOL MAIN OFFICE. Do NOT give physical packets to the coach!
